TILA Disclosures and Forms

NACUBO has collaborated with NASFAA to create these customizable disclosure forms and a customizable self-certification form for members to use. These were created by adding form fields to the model forms provided by the Federal Reserve Board in the final rules published August 14, 2009. Supplement I to Part 226 (Official Staff Interpretations) and Subsection F of the final rules explain how schools should properly use these forms.
